Your Form 1095-C, Employer-Provided Health Insurance Offer and Coverage, will be available for you to access electronically, you can view and print it in Workday as follows:

路 Select the 鈥淏enefits and Pay鈥 Application from the Menu
路 Click on 鈥淏enefits鈥 and select 鈥淢y ACA Forms鈥
If you are unable to access your form in Workday and would like to request a paper copy, you may do so by:
